Now that you have scheduled your appointment, it will save time to download two forms. The forms are a new patient encounter form and a medical history form. They are needed when you arrive in our facility. To have them already filled out will save time and give you more time with your surgeon.
You will need Acrobat Reader, a free software program to view the forms. When your acrobat reader program opens, print the forms on your printer. Fill out the forms and bring them with you to your scheduled consultation.
